Description
Provides a combination of services and supports to an adult primary caregiver who provides care for a functionally dependent older adult at home. Services include benefits counseling, caregiver training and education. Financial reimbursement may be provided for ongoing care giving expenses, home modifications and assistive devices, based on qualifications.
