Residents of Allegheny County, within select zip codes, or residents of Westmoreland County; Women who are pregnant, new parents, or families who have children up to the age of 18 months old; Partners of the mothers receiving services.

Description

Offers home visiting as a model of support for pregnant women and their families through 18 months postpartum. Families meet monthly with community health workers to improve maternal and child health through education, advocacy, case management, and referrals to community-based services. Eligible participants may receive diapers, books, educational toys, and other household items.
