Eligibility

A) Household income should be at or below 250% of the Federal Poverty Level B) Live or works in Centre County. C) Does not have insurance coverage for the specific services you are applying for. D) Full-time students at Penn State University or Lock Haven University are not eligible for CVIM services.

Description

Includes professionals in social work, psychology, counseling, and psychiatry who review client cases and make treatment recommendations. The behavioral case manager serves as a liaison connecting the IBHS team and the client’s medical providers and nursing professionals to facilitate comprehensive care.
