Provides adults with intellectual and developmental disabilities to live in the community of their choice in the private family home of their dedicated Lifesharing providers called Mentors, who provide the individuals we serve with: A) 24-hour home-based monitoring B) Medication management oversight C) Community integration/structured activities D) Life skills development E) Activities of daily living management F) Transportation G) 24/7 on-call support
