A) Under the age of 62 B) Someone in the household must be disabled. In addition, individuals will also be required to show proof of disability when submitting the application. This can be done either through the Disability Form provided with the application or by providing a copy of their Social Security disability award letter. C) Homeless, Risk of being Homeless, Institutionalized, or at Risk of being Institutionalized D) All applicants must provide a copy of a photo ID, such as a PA driver's license, passport, or military ID, for all adult members. Also, a copy of Social Security cards and birth certificates for each person on the application when you turn in your application.
