Eligibility
Persons who are homeless, at-risk of homelessness or living in
substandard housing conditions. Must have a diagnosed mental health
illness and be enrolled by Service Access and Management

Monday through Friday, 8:30am to 4:30pm

Description
Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing. In some cases, financial assistance such as first month's rent, a security deposit and/or basic household needs is also available through PATH.
