Eligibility

Services are available to clinically-eligible individuals age 55 and older who live in the Community LIFE service area and are able to live safely at home with the support of the LIFE program.

Fees

No fee for individuals who are eligible for Medicaid Monthly premium for individuals who are eligible for Medicare but not Medicaid Sliding scale private pay available for individuals who are not eligible for either; call for details.
