Most programs are provided to those who are 60 years of age or older. Assessments completed for individuals applying for waiver programs or facility placement.

Fees

Care Management and Level of Care Assessments are provided at no cost. Program fees may vary. Consumers may be eligible for Cost Share or an income-based fee scale. Other payment options may include services provided on donation basis (or no fee), payment through an outside agency (VA or MH/ID/EI), or private pay.
