Eligibility

A) Individuals diagnosed with a serious mental illness and able to provide documentation B) Must have documentation of sufficient income that proves that the individual can sustain the desired living situation, maintaining rent on the residence, for at least 90 days C) Individuals who are currently homeless or at imminent risk of homelessness

Application process

Dial 2-1-1: Individuals/families must have a referral sent by a housing specialist to receive these services. Once a referral is received, a call, within (3) business working days, will be placed directly to the individual to follow up on the current information and determine if the individual meets the eligibility for services. At that point, an intake appointment is scheduled with the individual.

Description

Assists with the transition from homelessness through the PATH program. Provides rental assistance, including one month of back rent to prevent eviction. Offers security deposit assistance for a new residence. PATH cannot assist with payment if more than one month of back rent is owed.
