Dial 2-1-1: Individuals/families must have a referral sent by a housing specialist to receive these services. Once a referral is received, a call, within (3) business working days, will be placed directly to the individual to follow up on the current information and determine if the individual meets the eligibility for services. At that point, an intake appointment is scheduled with the individual.

Description
Assists with the transition from homelessness through the PATH program. Provides rental assistance, including one month of back rent to prevent eviction. Offers security deposit assistance for a new residence. PATH cannot assist with payment if more than one month of back rent is owed.
