Description

Helps older Pennsylvanians to live independently while receiving services and supports that meet the health and personal needs of the individual. Services may include: LIFE Center primary care, dentistry, emergency services, hospital care, laboratory/x-ray services, medical specialty services, nursing home care, nutritional counseling, occupational and physical therapies, prescription drugs, home care, meals, caregiver training, support groups, approved transportation, and respite care.
