Eligibility

Applicants must lack adequate income to pay for dental care, and meet one of the following criteria: A) Have a permanent disability B) Be age 65 or older C) Qualify as medically fragile

Hours

Office Hours: Monday through Friday, 8:00am to 4:30pm

Application process

Apply online or print and return a completed application and required documents by mail.

Fees

From time to time, people who can pay for part of their care may be asked to do so (for lab work, etc.)

Dental Lifeline Network

A national charitable organization solely dedicated to meeting the needs of citizens with physical, medical, and mental disabilities. An affiliate of the American Dental Association and state and local dental associations. The Foundation's programs have the same goal, helping as many most vulnerable citizens as possible receive dental health services.
