Application process

Referrals can be made from obstetrician, WIC, pregnancy centers, schools, and other community providers. Self-referrals are also welcome.

Description

Provides nurse home visits to eligible first-time mothers early in pregnancy until child is two. Nurse sets goals with mom to ensure healthy pregnancy and healthy child.
