Eligibility

Allegheny County residents age 60 and older, who are experiencing some degree of frailty in regard to physical and/or mental status that impacts daily functioning.

Monday through Friday, 8:30am to 4:30pm

Description

Provides assessment of needs and care management to coordinate long-term in-home services for people age 60 or over. After OPTIONS completes a needs assessment, individuals may be eligible for: A) Home Delivered Meals B) Home Support Services C) Personal Care Services D) And more services depending on need
