Description
Assists families in linking with the mental health system and to coordinate needed services for children/adolescents. Involves child and family centered Inter-agency Service Planning Team meetings where families meet with service providers, CASSP Coordinators, and others to plan for home, school, and community treatment options. For individuals needing a higher level of care, a case manager will assist the family in choosing a Residential Program that will best meet the needs of the child.
