Description

Provides care and support for individuals of any age and their families coping with serious illness, end of life or loss. Services are provided by a team of professionals, including doctors, nurse practitioners, nurses, social workers, aides, chaplains, bereavement counselors, complementary therapists and volunteers, with a focus on improving quality of life and patient dignity, while also providing family support. Hospice and palliative care are available for adults, teens and children with any serious illness. - Clinical staff on-call around the clock and 24-hour admissions - Physical, emotional and spiritual support to patients, caregivers and families - Care is provided at home, in a senior living facility, in a hospital and at Hospice & Community Care's Bob Fryer & Family Inpatient Center in Mount Joy. - Bereavement support is available through the Pathways Center for Grief & Loss for adults, teens and children, regardless of whether hospice care was provided for a loved one. with any serious
