Eligibility

Must be 18 years or older with a chronic psychiatric illness and a history of long-term hospitalization and/or have been identified by BH/DS as being at risk for inpatient of state hospital services, no threat to self or others, agree to other admission criteria

Description

Provides short term, intensive and individualized treatment services for the identified persons in order to successfully re-integrate them into the community. Facility consist for two 3 bedroom semi-detached homes and accommodates up to seven individuals. Program length ranges from 6-12 weeks depending on individual's needs. Offers: * Supportive Counseling * Therapy * Educational programs * Behavioral management * Crisis Intervention * Community Re-Integration Skills * Psychiatric Services * Medication management
