Description
Provides case management and homeless prevention assistance to eligible individuals who are homeless or in danger of becoming homeless and who also have severe mental illness. Meets with clients in the community and helps clients locate housing (temporary or permanent), financial assistance to avoid eviction, and links to mental health and substance abuse treatment.
