Eligibility

Youth and young adults between 14 and 26 years old Covered by or eligible for Medicaid (Medical Assistance) Have serious emotional disturbance; mental illness; or dual mental health diagnoses, mild intellectual and/or developmental disabilities

Description

Provides Certified Peer Specialists who help clients develop their own Individualized Service Plans and Crisis Plans; identify community resources, develop organizational skills for daily living, and plan social supports. Meets clients where they are most comfortable, in their home or community.
