Application process

Call 570-546-4209 to make an appointment. You must have a referral from your doctor to make an appointment for outpatient therapy services. This referral must come in the form of a prescription. Our office staff will work with you and your doctor to assure completion of the appropriate paperwork.

Muncy Valley Hospital, located in Muncy, is positioned to serve the eastern portion of the county.
