Eligibility

A) Adults (age 18 and older) B) Must have a primary psychiatric diagnosis C) Must be capable of self-preservation D) Have basic ADL skills E) Willing and able to participate in treatment

Completion of an internal referral form and participation in an interview conducted by Program staff.

Description

Provides services to individuals with a history of serious and persistent mental illness. The focus of the program is the teaching and development of skills and supports needed to address particular life goals. Goals can include medication compliance, tenant responsibilities, budgeting skills, apartment/household maintenance, community integration, self-care skills, and community mobility training.
