Eligibility
Individuals must reside in the LIFE Pittsburgh Service Area, be age 55 and older, and:
A) Meet eligibility level for nursing facility level of care
B) Be certified Medicaid-eligible by the Department of Human Services (County Assistance Office) for LIFE Pittsburgh or be able to self-pay
C) Be able to live in the community safely with services from LIFE Pittsburgh
Hours
Office Hours: Monday through Friday, 8:00am to 4:30pm
Service hours vary, with some available 24 hours/day, 7 days/week.

Call for an initial screening and to begin the intake process.
Fees
Service is provided free of charge for those eligible for Medicaid. Call for more details for private pay.
