Eligibility
Individuals determined to need placement in a nursing home or Domcare (PCBH) and are financially eligible for medical assistance
Hours
Monday through Friday, 8:00am to 4:30pm
Application process
Needs assessment will be completed by an Aging Care Manager to identified the consumer's needs, level of functioning and appropriate supports, and is based on the consumer's own choices.
