Resources for Human Development - Lehigh Valley Assertive Community Treatment

Multidisciplinary community treatment teams provide intensive, individual support to individuals diagnosed with a serious mental illness in both Lehigh and Northampton counties. Specific services include psychiatric assessment, medication education and monitoring, treatment for co-occurring disorders, behavioral therapy, psychosocial services and education. A separate, intensive/blended case management unit provides continuity of care for those individuals with serious mental illness residing in Lehigh and Northampton counties who no longer require the intensive level of multidisciplinary assertive community treatment team services. The Lehigh Valley ACT Permanent Supportive Housing program provides independent, scattered site housing funded through a McKinney-Vento grant to chronically homeless individuals with serious and persistent mental illness.
