The goal of Medical Case Management (MCM) is to empower persons who are living with HIV to maintain optimal physical and emotional health through ongoing assessment of needs, linkage to medical care, retention in medical care, treatment adherence, service planning and monitoring, coordination of services and advocacy.Our Medical Case Managers meet with their clients at least once a month to update goals and to determine progress. Case managers meet with clients in the office, in their homes, or any community setting that is convenient. The following services are available for our MCM consumers: 340B Pharmaceutical Program, Transportation Assistance, Food Vouchers, Client Financial Fund, Support Group, and Peer Mentorship.

Champions a life of inclusion, opportunity, and equity for people with intellectual and developmental disabilities through advocacy, education, and support.

Serves as an advocate for Veterans involved in the criminal justice system to help them avoid unnecessary criminalization of mental illness and extended incarceration by connecting Veterans to VA mental health and substance abuse services.

Creates an environment that fosters the growth and development of youth using assets within the community, schools, families and peer groups to prevent problem and risky behaviors.

Provides services and resources to blind and visually impaired people. Services include non-medical information and referral, the ACB Braille Forum (available in braille, large print, digital cartridge, email, online, and podcast), the ACB E-Forum (available via email, online, and podcast), legislative and governmental monitoring, scholarship assistance, and public education.

Provides information, referral, and advocacy for individuals with autism. Offers inclusive community opportunities for community outings.

Provides advocacy services for individuals with disabilities. Provides legal services in cases of disability employment discrimination, special education, and disability rights.

Offers free legal help for low-income and vulnerable residents. Services may include bankruptcy, debt relief, consumer law, child custody and dependency issues, disaster legal aid, elder law, expungements, pardons, and record sealing. May also cover government benefits issues concerning Unemployment Compensation, SSI, SSDI, TANF (Temporary Assistance for Needy Families), SNAP (food stamps), Medical Assistance (Medicaid), and Child Health Insurance Program (CHIP). Housing issues such as landlord-tenant, eviction, habitability, public or subsidized housing, rent-to-own, homeownership, mortgage foreclosure, property tax sales, reverse mortgages, and tangled title; utility problems, and veterans benefits and discharge upgrades.

Provides a monthly Quality Enhancement meeting that provides opportunities to develop good leadership skills and to explore self-directed services. It also give the consumers the opportunity to participate in the policy and decision-making process, while offering training to direct support staff. This group welcomes people from the general community at a small charge if the individual is not supported through a Community Interactions' residential or day service.

Provides advocacy and professional support to ensure that public education is a reality for all. Focuses on underserved students, including students in poverty, students of color, students with disabilities, English learners, LGBTQ youth, students experiencing homelessness, and students in the foster care and juvenile justice systems.

Offers a wide spectrum of programs and services to meet the needs of the local disability community. Services include: *Disability Advocacy *Person Centered Counseling *Voter rights *Peer Support *E-sports *Technology Center

Provides 24-hour hotline support, legal advocacy, medical advocacy, accompaniment, trauma-informed empowerment/options counseling, support groups, Information and Referral, community prevention, and awareness education.

Supplies health kits, blankets, and sweatshirts to newly arrived workers; arranges churches to visit work camps; meets emergency needs such as transportation, translation, and help with prescriptions; and leads Bible studies and worship services.

Provides advocacy and facilitates the process of submitting comments for potential parole release and clemency hearings.

Provides ongoing empowerment counseling, tailored to individual needs, to victims, children and significant others. This includes crisis, economic and educative counseling. Additionally, support groups are held regularly for adults or teens who are survivors of domestic violence, rape, sexual assault and sexual harassment. The Barbara J. Hart Justice Center provides legal assistance to program participants, including representation, consultation and court preparation. Services include safety planning, Protection From Abuse orders, child custody, child support, divorce, nuisance ordinances from landlords, evictions, consumer debt, banking and tax issues, immigration issues, and employment cases.

Provides assistance to veterans and their families in obtaining benefits and services earned through military service and provided by the Department of Veterans Affairs (VA) and other government agencies.

Provides outreach concerning its program services to the American people generally and to disabled veterans and their families, specifically.

Providing a structure through which disabled veterans can express their compassion for their fellow veterans through a variety of volunteer programs.

Provides a community organizing model that puts community at the front of criminal defense.

Operates as a civil rights organization, participates in voter registration, criminal justice issues, housing, veterans and armed services, health, religious, legal redress, education, candidate forums, women issues, ACT, and etc.

Provides BHRS (Behavioral Health Rehabilitation Services)/IBHS (Intensive Behavioral Health Services) mental health services as well as respite care services for ages 2 to 21. Offers early Intervention and special instruction services, which provides 1-on-1 support for ages 0 to 3.

Offers residential homes, residential support, supported employment, self-advocacy, day services, and habilitation.

Assists individuals who are incarcerated or detained in prisons, jails, immigration detention centers, or other institutional settings by providing information, legal advice, advocacy, and representation of civil rights matter issues related to their conditions of confinement. Provides assistance in: A) Medical care and mental healthcare B) Freedom from retaliation and freedom from assault and sexual abuse C) Personal safety and freedom from assault and sexual abuse D) Access to the courts E) Religious accommodations and disability accommodations F) Clean, safe, and humane living conditions

Provides advocacy services that help families and self-advocates understand and navigate the mental health/intellectual and developmental disability service systems. The Arc's education advocacy services help parents, guardians, and students develop a plan of action for obtaining a free and appropriate education and helping students make meaningful progress on their educational goals. Provides an information line for families to call and ask questions.

Provides community educational sessions as well as one-on-one counseling on Social Security and Medicare benefit planning, long-term living planning, check writing and budgeting, and fiduciary bookkeeping. Helps to develop individualized plans to meet the needs of each client.

Provides forensic child interviews, medical exams, and services to reduce the trauma children experience during the investigation of abuse allegations.
