Offers services for individuals with substance use disorders. Provides screening and level of care assessment for substance use disorder, referral to treatment, and support until admission if space is not available in an appropriate setting immediately. Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.

Provides Early Intervention Service (EIS) coordination for Allegheny County families. Connects children in need with intervention services provided at their home or day care location.

The goal of Medical Case Management (MCM) is to empower persons who are living with HIV to maintain optimal physical and emotional health through ongoing assessment of needs, linkage to medical care, retention in medical care, treatment adherence, service planning and monitoring, coordination of services and advocacy.Our Medical Case Managers meet with their clients at least once a month to update goals and to determine progress. Case managers meet with clients in the office, in their homes, or any community setting that is convenient. The following services are available for our MCM consumers: 340B Pharmaceutical Program, Transportation Assistance, Food Vouchers, Client Financial Fund, Support Group, and Peer Mentorship.

Aids those individuals seeking recovery from substance abuse issues and to access quality treatment for their disease, while at the same time promoting the benefits of recovery.

Offers behavioral health consumers that are active in their treatments to utilize a case manager to identify and link to available community resources, coordinate various services they may have and to monitor their overall achievement on their recovery goals.

Provides assessment of the individuals need and coordination of resources and services for the consumer.

Provides services at an intensive outpatient and outpatient level of care. Offers individual, group, and case management services. Provides childcare and transportation to and from the Consortium. In addition, a Family Service Coordinator provides support and resources for the consumer's children.

Provides permanent supportive housing to persons with medical disabilities, and RSVP, which serves chronically homeless families. Both offer intensive weekly case management and monthly workshops.

Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing. In some cases, multiple months of rent may be paid along with a security deposit. Other supports, include furniture, household supplies, payments for accessing documents and other unique needs.

Provides permanent supportive housing assistance. Housing is provided at various apartment locations. Services include: Emergency Shelter, Permanent Supported Housing, Case Management, Life Skills Training , Outreach and Advocacy, Peer Support , Vocational/Educational Support , Participant Savings Plan.

An in-home assessment is completed with an older person to determine individuals needs. A plan is developed that provides the necessary support to meet these needs so the individual may remain at home. A care manager may be assigned to provide on going support and assistance.

Assists in the development of family management and parenting skills. Focuses on positive behavior support, relationship building, routines, and more.

Offers mental health assessments, referrals, and linkages to formal and informal services.

Provides classes and case support for Deaf refugees, immigrants, and asylees in Southeastern Pennsylvania. Classes include learning ASL, English, learning about the USA, and preparation for citizenship tests.

Provides Medical Case Managers who work in partnership with clients to remove practical and emotional barriers to primary medical care, access to medications, housing, benefits and entitlements, and transportation services.

Offers the following services to eligible elderly or disabled clients: assistance with personal care and hygiene; homemaker services such as light housekeeping, grocery shopping, and meal preparation; transportation; respite services; companion services; and case management. Also offers the Senior Health Specialist Program through which registered nurses provide health screenings and counseling, disease management education, nutrition education, and medication review for senior clients.

Provides reentry support and case management services for formerly incarcerated individuals with severe mental illness.

Provides case management services to survivors of human trafficking and sexual exploitation.

In addition, they have a Certified Spanish Interpreter on site to help facilitate optimal communication with patients, care and management of diabetes and other chronic illnesses, including transitioning from hospital to home through specialized office visits.
