Services are provided by a Registered Nurse and in the eligible persons home environment. The PA Elks Nurse can access needs and help access equipment, provide in-home medical case management, assist with finding activities and supports within the community.

Pennsylvania MENTOR’s dedicated team is available 24/7 to respond to emergency placement calls for children and adults. One of our experienced professionals will be there to answer calls and begin the placement process after office hours and on weekends and holidays.
