Provides assessment of the individuals need and coordination of resources and services for the consumer.

Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing. In some cases, multiple months of rent may be paid along with a security deposit. Other supports, include furniture, household supplies, payments for accessing documents and other unique needs.

Provides funding and administrative oversight for services in the community that support people and their families living with serious and persistent mental illness, children and adolescents with a mental health diagnosis, individuals with intellectual disability, individuals with Autism Spectrum Disorder and children birth to age three (3) for early intervention services. Enable youth to improve their functioning in the home, school and community by providing structured treatment services during the afternoon and evening hours. Provides therapeutic activities such as: social skills activities, sharing and listening activities, partner time activities, therapeutic recreation, homework time, community field trips and family outreach.

Provides information and support concerning mental and emotional illnesses, including guidance to mental health services, education, family and individual advocacy as well as referrals to local psychiatrists, psychologists, counselors, and therapists. Offers advocacy and PEER support for individuals with a mental illness and their families. Also offers support for families of children with Individual Education Plans in schools. Conducts educational presentations for schools, clubs, employers, businesses and community groups on topics related to mental health. Mental Health services targeted for adults who are seriously mentally ill and children/adolescents who are seriously emotionally disturbed or at-risk of becoming so. Health professionals help clients to stabilize and educate about medication; to support and strengthen interpersonal relationships; to find and maintain meaningful employment; and to develop day-to-day skills such as money management, housekeeping, grocery shopping, meal preparation, laundry, using public transportation, and utilizing community-based services such as dentists and PCP's.

SAM is an organization working with the community to provide case management services for consumers and individuals enrolled in early interventions, intellectual disability and mental health.
