As well as overcome other obstacles of navigating care, including access to physical and behavioral healthcare, treatment for SUD, transportation, and connection to community resources. Enrolled individuals will receive case management for a period up to 2 years by a master’s level social worker (BHCM) and a certified recovery specialist (CRS).
