The goal of Free2BMom is to improve outcomes for mom and baby by reducing stigma and helping new moms or pregnant individuals face all the normal challenges of being a new parent. As well as overcome other obstacles of navigating care, including access to physical and behavioral healthcare, treatment for SUD, transportation, and connection to community resources. Enrolled individuals will receive case management for a period up to 2 years by a master’s level social worker (BHCM) and a certified recovery specialist (CRS).

Assists children and adolescents with a variety of behavioral health/medical needs and their families by building a strong network of community resources and services, linking clients with needed services in the community. Provides a variety of supportive services including advocacy for children and their families; negotiation of complex systems such as special education and social services; assistance for families in maintaining and finding access to basic living needs and skills. Provides on-site crisis intervention, with on-call Case Management available 24 hours/7 days a week.

Offers a variety of free and confidential support services for individuals and families facing an unplanned pregnancy. Services include free pregnancy tests, limited ultrasounds, and consultations to discuss all options like parenting, adoption, or abortion.

Offers an analysis of client's needs and development and implementation of a service plan to meet client's needs for maintaining independence. Provides confidential counseling by trained staff about problems with finances, family, health, etc., and coordination of human services.

Connects at-risk individuals to community resources to reduce both medical and social barriers to care, like employment, housing and transportation, for individuals with complex health needs.

Provides long-term case management services to individuals and families who need assistance securing and maintaining basic needs (housing, food, utilities, medical, etc.), navigating the human services system for appropriate resources, and following client-centered goals and service plans to improve overall self-sufficiency.
